Calico providers are in-network with the following insurance companies:
Blue Cross Blue Shield
Providence Health Plan (Providence Behavioral Health)
United Health Care (United Behavioral Health/UMR)
For clients whose insurance company has not contracted with Calico, your insurer will cover a portion of the cost of the visit according to your policy’s benefits for out-of-network providers. Unless other arrangements are made in advance, we will require that you pay in full for the service at the time of service. We will be happy to submit a claim on your behalf to your insurance company as a courtesy. Any portion of the fee covered by your insurance would then be reimbursed directly to you from the insurance company. The fees for services out of network are listed on our Fee Schedule. We always encourage clients to contact their insurance company directly to find out what their mental health benefits are, since clients will be responsible for whatever insurance does not reimburse for. However, we understand that this process can be confusing and we are happy to help collect that information from your insurance company. To do a benefit check you will need to call and provide us with the following information:
Name of policy holder (if different)
Date of birth
Name of Insurance Co.
We also accept clients who are not using insurance at the rates listed in our Fee Schedule.
